Coldspring Transitional Care Center
Cold Spring Transitional Care Center provides short and long-term care for individuals with rehabilitative and nursing care needs. The facility offers speech-language therapy, occupational therapy, and physical therapy for individuals requiring rehabilitation services.
Achievement Overview:

Manage a caseload of adult and geriatric patients with neurogenic communication disorders concomitant to neurogenerative diseases and acquired brain disorders

Administer and interpret the screener to assess cognitive status of patients

Collaborate with patient and caregiver(s) to determine functional goals

Conduct bedside swallow and oral mechanism examinations, and refer for a MBSS as warranted
-
Implement exercises to strengthen swallowing musculature, instruct in strategies and environmental modifications, and determine appropriate diet texture consistencies for individuals with dysphagia

Collaborate with an interdisciplinary team comprised of occupational therapy, physical therapy, dietetics, and nursing on a consistent basis

Experience with tracheostomy care, including implementation of Passy Muir valve and cap to facilitate voice and swallowing exercises
